Purpose

The Federal Office of Rural Health Policy is offering funding to promote the delivery of healthcare services to rural, underserved populations in the rural Northern Border Regional Commission (NBRC) Region of Maine, New Hampshire, New York, and Vermont.

Program objectives include:

  • Expand the delivery of new and enhanced healthcare services exclusively in the Northern Border rural communities
  • Deliver healthcare services through a strong consortium, in which every consortium member organization is actively involved and engaged in the planning and delivery of services
  • Utilize community engagement and evidence-based or innovative, evidence informed model(s) in the delivery of healthcare services
  • Improve population health, and demonstrate health outcomes and sustainability
Eligibility

Eligibility requirements:

  • Physically located within Maine, New Hampshire, New York, or Vermont
  • Domestic public or private, nonprofit or for-profit entity with demonstrated experience serving, or the capacity to serve rural, underserved populations
  • Represent a consortium of participants that includes at least 3 or more healthcare provider organizations
