  • Need: Improved approach in addressing the behavioral health and primary care disparities of Indiana’s underserved rural counties by increasing access through health insurance enrollment and providing health literacy education.
  • Intervention: A network was established to address behavioral health integration, which included the use of dually trained community health workers (CHW) and certified health insurance enrollment navigators.
  • Results: Creation of a certification process, the “Indiana Integrated Care Training Program: Community Health Worker/Certified Recovery Specialist,” for CHWs and cross-training of mental health peers to also work as CHWs

  • Need: Backup coverage for primary care providers in rural Montana.
  • Intervention: A network of healthcare facilities developed a primary care telemedicine program.
  • Results: While the technology worked for primary care telemedicine, and patients were open to the process, primary care telemedicine did not become a long-term solution in this Montana region due to low patient volume and a lack of telemedicine provider availability.

  • Need: To help ensure the viability of and improve quality at 9 rural, independent hospitals serving 11 Utah counties.
  • Intervention: A network organization was created to allow member hospitals to communicate, network, and undertake projects together.
  • Results: Members took advantage of cost savings, education, and networking opportunities through group projects and programs.
