Module 7: Program Clearinghouse

The HRSA Federal Office of Rural Health Policy funded rural communities to implement services integration programs as part of the 330A Outreach Authority program. This program focuses on expanding access to healthcare services in rural areas. Examples of current 330A Outreach Authority grantees and other promising programs that developed a services integration program in a rural community are provided below. Services integration program model information is available in Module 2.

  • Bleckley County Board of Education
    Project Title: South Georgia Regional Prevention Coalition
    Synopsis: This coalition increases access to care for children via school-based health centers and telehealth that allows for consultations with specialists.
  • Giles Free Clinic
    Project Title: Giles County Health Network G-SMILE Program
    Synopsis: This program promotes the integration of dental and primary care through co-location of services.
  • The Health Enrichment Network
    Project Title: Growing Up Fit Together2
    Synopsis: This program provides students with health and nutrition education, opportunities to engage in physical education, and access to oral health services.
  • Integrating Professionals for Appalachian Children (IPAC)
    Project Title: Community HUB/Care Coordination Pathways Program
    Synopsis: This network helps connect at-risk individuals to health and social services by serving as a Community HUB that bridges organizational silos.
  • Kreider Services, Inc.
    Project Title: Florissa
    Synopsis: This organization offers a one-stop shop for the assessment and treatment of children and families facing developmental, behavioral, and social/emotional issues.
  • Lake County Tribal Health Consortium, Inc.
    Project Title: Linkages: Expanding Behavioral Health for Native American Families through Integrated Services
    Synopsis: This program increases access to behavioral health services for pregnant Native American women through co-location of services and care coordination.
  • Santa Cruz County Mariposa Community Health Center
    Project Title: Adolescent Wellness Network
    Synopsis: This network increases access to care for children through a school-linked health program and coordination with health and human service providers in the community.
  • Paris-Henry County Healthcare Foundation, Inc.
    Project Title: West Tennessee Delta Consortium
    Synopsis: This program is increasing access to care for children through school-linked health services that include mobile health clinics.
  • St. James Healthcare Foundation
    Project Title: Butte Community Diabetes Network
    Synopsis: This program integrates resources for community members with diabetes through case management and a network referral system.
