Purpose

The New York Rural Health Network Development Program offers grants for rural healthcare providers to improve access to care, enhance coordination of services, increase efficiency of service delivery, and introduce needed community services through the formation and operation of rural health networks.

The objectives of the program include:

  • Encourage and strengthen cooperation through affiliations, coalitions, or partnerships that enhance and coordinate needed healthcare services in rural areas
  • Increase community involvement and locally-generated, innovative solutions to health systems issues in rural areas
  • Support the development and continued vitality of comprehensive, cost-effective healthcare systems serving rural areas
  • Promote the financial viability of rural healthcare providers in the existing healthcare economy
  • Increase access to healthcare services and fill gaps in service delivery
  • Increase the numbers of healthcare personnel trained and practicing in rural communities
  • Strengthen the accountability of local providers for improvements in key public health and/or healthcare outcomes in the communities they serve
  • Foster improvements in the economic position of the local community
Eligibility

Applicants must meet all of the following requirements:

  • Be nonprofit organizations, municipalities, or a units of a municipality
  • Must serve residents in a rural area of New York state
  • The rural health network must be composed of at least 3 members that are separate, existing healthcare providers, human service agencies, community-based organizations and/or educational institutions, each of which has its own Federal Tax Identification Number and is located in the area proposed to be served by the rural health network
  • Demonstrate the existence of formal agreements among the network members to participate in network activities
  • Governed by a Board of Directors that is representative of its network members and the community that is being served
