Frontier Community Health Integration Project Technical Assistance, Tracking, and Analysis
The Frontier Community Health Integration Project Technical Assistance, Tracking, and Analysis grant will award funding to an organization for the purpose of providing technical assistance, site implementation assistance, and other tracking and analytic activities to support the Critical Access Hospitals participating in the Frontier Community Health Integration Project (FCHIP) Demonstration.
The grantee will assist FCHIP providers in identifying potential new approaches to health care delivery, reimbursement, and coordination in sparsely populated areas. The grantee will also be responsible for completing individualized needs assessments of participants' capacity and collect information on key policy issues faced by frontier providers.
Eligible applicants include public, private, and nonprofit organizations, including faith-based and community organizations, as well as federally-recognized tribal governments and organizations.
Number of awards: 1
Total funding available: $500,000 per year for years 1 and 2 and $450,000 for year 3
Project period: September 1, 2017 - August 31, 2020
