Purpose

Iowa Community Health Needs Assessment and Health Improvement Plan Unmet Needs Grant awards funding to local boards of health in Iowa to develop and implement sustainable approaches to addressing unmet community needs that were identified in the state community health needs assessment and health improvement plan.

Eligibility

Only local boards of health in Iowa are eligible.
