Transforming Clinical Practices Initiative (TCPI) Support and Alignment Networks (SANs) 2.0 will award funding for the establishment and operation of support networks, which will offer support, technical assistance, and expertise to spread healthcare delivery system reform knowledge to the entire TCPI community. TCPI SAN 2.0 will prepare clinical practices for the accelerated adoption of Alternative Payment Models (APMs) on a large scale and at low cost before 2019.

TCPI SAN 2.0 will identify, enroll, and provide technical assistance to clinical practices in order to reduce Medicare, Medicaid, and CHIP program expenditures. They will assist practices through the various phases of the transformation process and help them to align their practice to participate in the most suitable APM options that were authorized under the Medicare Access and CHIP Reauthorization Act of 2015 (MACRA).


Organizations, including health care delivery systems and health care delivery plans, that are:

  • Currently providing quality improvement support to a large number of clinicians
  • Multi-regional or national in scope
  • Involved in creating evidence-based guidelines for clinical practice
  • Using measurement through clinical registries and electronic health records effectively
  • Committed to improving safety and person and family engagement

Medical professional associations and specialty societies may also apply. Organizations that applied and/or were awarded as a TCPI-PTN or SAN previously may not apply.
