The RCHN Population Health Initiative will award funding to support a representative and committed group of health center grantees and partners that are planning or are engaged in local or state initiatives to engage in "health neighborhoods" that support systems for population health management.

Applicants should identify a health condition or health services situation, a population of focus, and an intervention plan. Applicants must identify key partner organizations that will work to create an impact in the health of a community.


Entities that are currently designated by the Health Resources and Services Administration Bureau of Primary Health Care as Federally Qualified Health Centers or FQHC Look-Alikes, as well as state primary care associations (PCAs).

Amount of funding

The Foundation expects to fund up to 5 awards under this program, with grant awards of approximately $125,000 per award, including up to 10% for indirect costs. The project period is 2 years.

Application process

According to the application instructions, applicants must submit an online letter of interest. Selected applicants will be invited to submit a full proposal, with a deadline 4 weeks after notification.
