Strategies to Increase Delivery of Guideline-Based Care to Populations with Health Disparities (R01) will award funding for innovative, multi-level studies to test systems, infrastructures, and strategies that will accelerate the adoption of guideline-based recommendations into clinical care relevant to heart, lung, and blood diseases, and sleep disorders.
Practice settings could include:
- Federally Qualified Health Centers
- Safety-net facilities
- Community health centers
- Public hospitals
- Other practice settings treating populations with disparities
Health disparities populations include, but are not limited to:
- Rural-dwelling patients
- Medically underserved individuals
- Racial and ethnic minorities
- Low income groups
Eligible applicants include:
- Institutions of higher education
- Nonprofit organizations
- Private businesses
- Independent school districts
- Public housing authorities
- Tribal organizations
- Faith-based, community-based, and regional organizations

Application budgets are not limited, but need to reflect the actual needs of the proposed project. The maximum project period is 5 years.
