The Transforming Clinical Practice Initiative (TCPI) provides technical assistance, education, and support for healthcare providers in rural communities who are preparing for and participating in value-based payment models.

Available assistance includes:

  • Setting up billable care coordination service
  • Redesigning practices to manage population health
  • Creating infrastructure to prepare for alternative payment models
  • Increasing revenue

Eligible participants are rural healthcare practices that agree to attend trainings and predefined meetings with key staff, physicians, and CEOs.

The National Rural Accountable Care Consortium will work with at least 500 rural communities over the course of a 4-year project that began in September, 2015.
