The Transforming Clinical Practice Initiative (TCPI) provides technical assistance, education, and support for healthcare providers in rural communities who are preparing for and participating in value-based payment models.

Available assistance includes:

  • Setting up billable care coordination service
  • Redesigning practices to manage population health
  • Qualifying as a Patient-Centered Medical Home
  • Increasing revenue

Eligible participants are rural healthcare practices that agree to attend trainings and predefined meetings with key staff, physicians, and CEOs.

The National Rural Accountable Care Consortium will work with over 500 rural communities.
