Healthier Wisconsin Partnership Program (HWPP) Change Incubator Funding will support diverse community-based partnerships to build their capacity to carry out community health improvement initiatives.

Change Incubator focus areas include:

  • New partnership development and action planning
  • Strategic growth and sustainability planning for existing partnerships
  • Development of communication plans and materials to communicate impact

All projects must result in an end product that will guide the partnership moving forward.


Applications must be submitted by a partnership of at least two Wisconsin-based, health-related entities representing diverse organizations and perspectives invested in improving the health of Wisconsin communities. One partner must be a nonprofit, IRS tax exempt 501(c)(3) or government organization, have an EIN number, and have the capacity to serve as the fiscal agent for the partnership.
