Funding for rural primary care physician practices in Georgia to improve quality of care as well as patient and provider experience of care using the Patient Centered Medical Home (PCMH) healthcare delivery model.


Eligible providers and practices must be:

  • Located within rural counties in Georgia
  • Primary care physician practices, such as general practice, family practice, internal medicine, pediatrics, and OB/GYN
  • Single site practices
