The Community Catalyst Grant Program provides funding to support creative health equity approaches designed with the goal of actively reducing gaps in access, outcomes, and opportunities in Wisconsin.

The program is designed to support projects that are:

  • Focused on health equity
  • Community-driven
  • Asset based
  • Innovative
  • Have expected impacts beyond the grant term

Wisconsin-based, nonprofit 501(c)(3) organizations, tribes, or government entities are eligible to apply.
