Purpose

AMA Foundation Community Health Grants support evidence-based projects that demonstrate focused, measurable results in community health and medical education programs. Grant funding will support community-based organizations, such as free health clinics, that provide services for vulnerable and underserved populations, especially services that target the prevention and reduction of type 2 diabetes and hypertension.

The program goal is to increase health equity and improve health outcomes among vulnerable populations utilizing evidence-based models of care.

Eligibility

Eligible applicants are community based organizations, including free clinics, that meet the following criteria:

  1. U.S. based nonprofit organizations with 501(c)(3) status
  2. In operation for over 3 years
  3. Annual operating budget less than $3,000,000 (if the agency has multiple sites, the location where the project will be implemented must have an annual budget less than $3,000,000)
  4. Provide services in the continental U.S.

Up to $50,000, renewable for up to 3 years.
