Grants to support rural service providers, rural health clinics, health professional shortage areas, and medically underserved areas and populations in Virginia in the areas of:

  • Workforce development
  • Telehealth
  • Substance abuse
  • Behavioral health
  • Community paramedicine

Project examples include:

  • Training community health workers on chronic disease management and have them provide patient education during EMS calls for non-emergent patients
  • Collaborate with local law enforcement to provide case management and resources for substance abuse clients instead of processing clients into the criminal justice system during the booking process
  • A behavioral telehealth program using the local health department to facilitate telehealth referrals

Eligible applicants are public agencies, tribal organizations, and private nonprofit organizations in Virginia that are:

  • Designated rural service providers
  • Rural Health Clinics
  • Health Professional Shortage Areas
  • Medically underserved areas and populations

Award ceiling: $35,000
Award floor: $1,000
Project period: 1 year
