The Patient-Centered Medical Home (PCMH) Recognition Quality Awards recognize health centers' commitment and investment in quality improvement activities. HRSA provides financial and technical support to help health centers achieve accreditation and PCMH recognition.


Health centers that have PCMH recognition by July 1, 2019 are eligible to receive a 2019 Health Center PCMH Recognition Quality Award.

Amount of funding

The amount of the PCMH recognition award in fiscal year (FY) 2019 will be determined based upon the availability of federal funding.

Application process

Follow the instructions listed on the program website.

  1. Review the PCMH recognition status listed on the PCMH Verification Table.
  2. If the information is correct, no further action is needed.
  3. If the information is incorrect, submit an update to PCMH recognition on HRSA's Electronic Handbooks (EHBs) by July 5, 2019. Log into the EHBs grant folder and choose the "HRSA Accreditation/PCMH Initiative" link.
  4. For health centers initiating or renewing PCMH recognition for fiscal year 2020, submit an online letter of intent. HRSA will provide technical support to help health centers achieve accreditation or PCMH recognition.
