The Colorado Health Foundation is providing funding to support safety net clinics in Colorado in receiving technical assistance to improve their clinical and business capacity to offer high-quality, team-based comprehensive primary care.

Priority will be given to projects that serve patients in rural and/or frontier communities in Colorado.


Eligible applicants include:

  • Federally Qualified Health Centers
  • Rural Health Clinics
  • Community safety net clinics
  • School-based health centers
  • Community mental health centers with integrated primary care services
Geographic coverage
Amount of funding

Up to $125,000 over a 2 year period.
