Purpose

The New Jersey Rural Health Community Initiative Grant provides funding to decrease morbidity and mortality rates among vulnerable rural populations in New Jersey through outreach, prevention, and education-based initiatives. Priority areas include chronic disease prevention and management, maternal and child health, cancer incidence and mortality, and behavioral health risk factors such as tobacco use and unhealthy eating habits.

Program objectives include:

  • Increase access to preventive care services and resources
  • Create and expand evidence-based initiatives that provide education and outreach on disease prevention and management and community care coordination
  • Cultivate sustainable partnerships with rural stakeholders to implement and evaluate public health interventions

Eligibility

Eligible applicants include:

  • 501(c)(3) nonprofit organizations
  • Community-based organizations
  • Faith-based organizations
  • Federally Qualified Health Centers (FQHCs)

Applicants should have experience and the ability to implement community-based programs for increasing awareness, education, outreach, and community care coordination in the areas of primary care, oral health, and/or maternal and child health with a focus on health disparities in the rural regions of New Jersey.

Geographic coverage
Rural regions of Atlantic, Burlington, Cape May, Cumberland, Hunterdon, Ocean, Salem, Sussex, and Warren counties in New Jersey.

Award ceiling: $20,000
Project period: 11 months

Application process

A link to the application instructions (RFA) is available in the Community Health Services, Office of Primary Care and Rural Health section of the program website.

Applicants are required to email a letter of intent to Damon.Koslow@doh.nj.gov by May 21, 2021.
