The Elder Wellness Enrichment Initiative Grant provides for nonprofit rural healthcare organizations and community-based organizations in Kentucky to create or enhance services for older adults that help them to age in place. Services must be innovative and potentially serve as a best practice to be shared in Kentucky and with other states. Preference will be given to projects that place an emphasis on nutrition.
Nonprofit healthcare organizations and community-based organizations serving elders in rural Kentucky are eligible to apply.
Applicants are required to collaborate with at least 1 of the following rural health stakeholders:
- Critical Access Hospitals
- Small rural hospitals
- Rural Health Clinics
- Federally Qualified Health Centers
- Local health departments
- Local EMS agencies
Award ceiling: $5,000
Project period: 6 months
