The Elder Wellness Enrichment Initiative Grant provides for nonprofit rural healthcare organizations and community-based organizations in Kentucky to create or enhance services for older adults that help them to age in place. Services must be innovative and potentially serve as a best practice to be shared in Kentucky and with other states. Preference will be given to projects that involve solutions for elder needs in response to COVID-19.


Nonprofit healthcare organizations and community-based organizations serving elders in rural Kentucky are eligible to apply.

Applicants are required to collaborate with at least 1 of the following rural health stakeholders:

  • Critical Access Hospitals
  • Small rural hospitals
  • Rural Health Clinics
  • Federally Qualified Health Centers
  • Local health departments
  • Local EMS agencies
