This opportunity provides funding to nonprofits and public organizations working to address regional needs related to school-based healthcare in Appalachian Ohio. Funding categories include:
- Collaboration grants - to share population health data with community stakeholders, leading to the future development of small-scale interventions focused on addressing health disparities

Implementation grants - to address the
connections needed between formal and informal health
system partners to implement school-based healthcare
interventions in local communities in the following
areas:
- Services to children interacting with the foster care system
- Access to dental care
- Health professional shortage areas in the region
- Food insecurity
- Childhood obesity
- Behavioral health
- Childhood immunizations
- Teen pregnancy and neonatal abstinence syndrome
- Others based on relevant data
Eligible applicants are public and nonprofit organizations, including public schools, in the Foundation for Appalachian Ohio service area.
Estimated total program funding: $50,000
Awards are expected to range from $5,000 - $15,000. Requests up to $20,000 will be considered.
