The Montana Healthcare Foundation will provide grants and technical assistance for Montana local and tribal health departments for planning, development, and implementation of organizational and community health system improvement plans.
There are currently 3 project options:
- Funding for the completion of a community health assessment, community health improvement plan, organizational strategic plan, quality improvement plan, workforce development plan, and/or performance management system
- Funding for the implementation or expansion of a health improvement program, implementation of a health improvement priority as identified in the community health improvement plan, implementation of an organizational development objective as identified in the health department's strategic plan, or the initiation/implementation of another health department project
- Direct, one-on-one mentoring and technical assistance to the awardee from a subject matter expert in projects such as building community collaboration, PHAB accreditation management, and Board of Health development
All Montana local and tribal health departments are eligible to apply.
Awards will either be no-cost technical assistance or grant awards depending on budget needs.

Applications are accepted on an on-going basis. Projects must be completed by July, 2023.
