The Community Mini-Grant program provides funding to develop, implement, and evaluate evidence-based or evidence-informed health outreach projects to reduce health disparities in rural and Appalachian communities in Kentucky.
Priority areas include:
- Cancer prevention, including nutrition, physical activity, and smoking cessation
- Reduction of obesity
- Supporting healthy lifestyle
- Chronic disease prevention or management, including for diabetes and cardiovascular disease
- Risk behavior prevention and reduction
- Substance abuse prevention and reduction
Community agencies and coalitions with 501(c)(3) status are eligible to apply. Health agencies and nonprofit organizations may serve as a designated lead fiscal agency.
Projects must target Appalachian communities in Kentucky and also demonstrate broad community support, including specific plans for involving community members as members of the target population, local health-related organizations, and local businesses.
