Purpose

The Oregon Rural Population Health Incubator program provides funding to Critical Access Hospitals (CAHs), Rural Health Clinics (RHCs), and nonprofit organizations to strengthen community health through the support of population health and health equity programs in rural Oregon. This new program was created by combining three previous programs, the Elder Service Innovation Grants, Rural Health Clinic Innovation Grants, and Rural Population Health Grants, and allows the Oregon Office of Rural Health to support grantees with larger grants for programs.

The goals of the Rural Population Health Incubator program are to:
  • Provide funding to create or maintain innovative programs that support population health, health equity and/or social drivers of health
  • Prioritize funding for programs that:
    • Serve a high-needs service area as outlined in the Oregon Office of Rural Health's (ORH) Areas of Unmet Health Care Need Report
    • Serve historically marginalized populations
    • Address a pressing community health need identified by the organization's or county's community health needs assessment
    • Bring together multiple community partners to address the specific population health need they have identified

ORH will provide coaching, mentorship, referrals to experts, and other resources to Incubator grantees, as well as a supportive and collaborative environment for Incubator grantees to share knowledge and learn from one another.

Eligibility

Eligible applicants include:

  • Critical Access Hospitals (CAHs)
  • Rural Health Clinics (RHCs)
  • Non-profit organizations
Organizations must also be implementing a program aimed at increasing community health, whether directly or indirectly, and should be located in a rural area in Oregon as determined by the Federal Office of Rural Health Policy eligibility using the Am I Rural? Tool.
