The Healthy Families Community-Based Perinatal Health Initiative will provide grants to develop and implement innovative models for integrating community-based maternal support services (COMSS) into perinatal systems of care. Models should be designed to improve pregnant and post-partum people's health outcomes and reduce racial and ethnic disparities. Rural and other minority populations are a priority.

COMSS are defined as social and supportive services that address social determinants of health (SDOH), such as health literacy; pregnancy, childbirth, and parenting education; cultural and linguistic diversity; exposure to trauma, housing; food; and transportation.

Projects should:

• Develop and implement the COMSS integration model
• Establish a collaborative partnership to support integration of COMSS
• Integrate Healthy People 2030 and other measures into the development of the COMSS integration model
• Implement a process and outcomes evaluation
• Identify, utilize, and disseminate best practices
• Develop a disparity impact statement

Eligible applicants are public and private nonprofit entities located in a state or territory, including:

• State, territory, county, city, township, and special district governments
• Federally recognized native American tribal governments
• Independent school districts
• Public, state controlled, and private institutions of higher education
• Public housing authorities/Indian housing authorities
• American Indian or Alaska native tribally designated organizations
• Nonprofit organizations with and without 501(c)(3) status
• Faith-based organizations

Award ceiling: $1,250,000 per year
Award floor: $975,000 per year
Project period: 4 years
Estimated number of awards: 10
Estimated total program funding: $10,000,000
