Closing the Gap with Social Determinants of Health Accelerator Plans
Funding to collaborate with multi-sectoral partners to develop an implementation-ready Social Determinants of Health Accelerator Plan to reduce disparities in health outcomes related to chronic disease in a tribal jurisdiction, community, or catchment area. Health disparity populations include socially, economically, geographically, and environmentally disadvantaged populations.

Great Plains Center for Agricultural Health Community-Track Pilot Project Grants
Grants for community-based projects working to prevent agricultural injury and illness through training and educational campaigns. Priority will be given to projects addressing work‐related stress and mental health, the prevention of slips/trips/falls and other traumatic injuries, and chemical safety.
