Closing the Gap with Social Determinants of Health Accelerator Plans Inactive
Funding to collaborate with multi-sectoral partners to develop an implementation-ready Social Determinants of Health Accelerator Plan to reduce disparities in health outcomes related to chronic disease in a tribal jurisdiction, community, or catchment area. Health disparity populations include rural and tribal communities, among others.
