community-based providers to improve coordination of care
between rural community health centers (CHCs) and the
Veterans Health Administration (VHA). Includes lessons
learned and recommendations to increase enrollment and improve
data collection, outreach, and education. Offers information
on developing a framework for clinical coordination between
VHA and rural CHCs at the local and national levels.
