White paper describing a super utilizer program design, implementation, operation, and assessment for rural hospitals to improving participant health, reducing ED visits, and decrease hospital charges. “Super utilizer” patients often present to the emergency department (ED) with chronic and low acuity health concerns that are inappropriately addressed in a ED setting designed to care for acute, episodic, and emergent health conditions.
