- Program Overview: The Bridge/El Puente works with Hispanic populations and underinsured/uninsured non-Hispanic populations to obtain medical care through partnerships with Federally Qualified Health Centers (FQHC), local primary care dentists and oral surgeons, and optometrists. This project focuses on reducing barriers to healthcare access and improving health outcomes through outreach, health education, and management of chronic conditions. Individuals that are diagnosed or considered at risk for a chronic condition are provided education and support specific to their condition and, when needed, chronic disease management (for respiratory disease, diabetes, cardiovascular disease, or breast and cervical cancer).
