  • Program Overview: The Northwest Rural Health Network (NWRHN) is a nonprofit network of 15 member rural health systems in eastern Washington. NWRHN shares resources, improves healthcare services, and promotes efficiency. NWRHN has a long-standing relationship with the Empire Health Foundation. NWRHN collaborates with Empire Health Foundation and Molina Healthcare to implement a care coordination model to improve the health status of patients with chronic disease across three rural counties.
