FCC: Promoting Telehealth for Low-Income Consumers
Jul 30, 2019 -- Notice from the Federal Communications Commission (FCC) proposing a 3-year pilot program under the Universal Service Fund (USF) that would support telehealth for veterans and low-income individuals to help them access healthcare services. The program would likely focus on people with chronic health conditions, opioid dependency, mental or behavioral health issues, and high-risk pregnancies. It will likely include rural and urban areas, but may award additional points to proposed projects serving certain populations such as rural and tribal areas. Comments on the proposed rule are due August 29, 2019.

HHS: Request for Information: Ensuring Patient Access and Effective Drug Enforcement
Jul 30, 2019 -- Request for information from the Department of Health and Human Services (HHS) on how to ensure legitimate access to controlled substances such as opioids while preventing diversion and abuse. The Ensuring Patient Access and Effective Drug Enforcement Act of 2016 (EPAEDEA) requires that HHS submit a report to Congress that incorporates feedback and recommendations from various stakeholders. Comments are due August 29, 2019.

Jul 29, 2019 -- Pre-publication notice from the Centers for Medicare and Medicaid Services (CMS) of a proposed rule that will be published August 14, 2019. Rural specific provisions include: proposing to allow opioid treatment programs (OTPs) to use Medicare telehealth for substance abuse counseling; requesting information on rural access to opioid use disorder treatment; updates to several payment codes for Rural Health Clinics and Federally Qualified Health Centers; proposed revisions to physician supervision of physician assistants to better align with state laws; a proposed Medicare ground ambulance data collection system that would include information on types of services provided in rural areas; proposed revisions to certified registered nurse anesthetists (CRNAs) to align pre-surgery evaluation and pre-discharge standards; request for feedback on small and rural practices participating in MVPs; a proposed update to the MIPS rural area definition correcting the file name; and requesting comments on Promoting Interoperability performance measures related to OUD prevention and treatment that would be applicable to clinicians in rural areas. Comments will be due September 27, 2019.

Jul 29, 2019 -- Pre-publication notice of a proposed rule from the Centers for Medicare and Medicaid Services (CMS) making changes related to End-Stage Renal Disease (ESRD) payments and Quality Incentive Program (QIP) requirements and updates to the Durable Medical Equipment, Prosthetics, Orthotics and Supplies (DMEPOS) payments, bidding, and ordering requirements. The proposed rule also requests information on changing the basis for the ESRD PPS wage index and other areas. Rural specific components include: an explanation of methodology for calculating rural and urban area wage indexes, summaries of comments related to rural ESRD facility costs, the number of rural ESRD and renal dialysis facilities and effects of 2020 proposed changes, and expected QIP impact on rural facilities. The proposed rule is scheduled to be published August 6, 2019. Comments will be due September 27, 2019.

Public Inspection: CMS: Medicare Program: Proposed Changes to Hospital Outpatient Prospective Payment and Ambulatory Surgical Center Payment Systems and Quality Reporting Programs; Price Transparency of Hospital Standard Charges; etc.
Jul 29, 2019 -- Pre-publication notice of a proposed rule from the Centers for Medicare and Medicaid Services (CMS) revising payments for the Medicare hospital outpatient prospective payment system (OPPS) and Medicare ambulatory surgical center (ASC) payment system and updating quality reporting requirements. Also proposes requirements for making hospital standard charges public and other revisions and requests comments on remedying litigation related to 340B drugs. Rural specific items include: proposed adjustments for Rural Sole Community Hospitals (SCHs) and Essential Community Hospitals (EACHs), minimum supervision requirements for Critical Access Hospitals (CAHs), a proposed wage index update, continued rural hospital payment adjustments, and an increase in the OPD fee schedule. The notice will be published August 9, 2019. Comments will be due September 27, 2019.

Study: Broadband a Boost for Rural Telemedicine Use
Jul 29, 2019 -- A new study shows that people in counties with poor broadband access are less likely to be treated via telemedicine. The researchers recommend prioritizing entirely rural counties for Federal Communications Commission (FCC) broadband funding to increase rural telemedicine access.

Jul 26, 2019 -- Notice from the Health Resources and Services Administration (HRSA) of new performance measures to provide data on Rural Communities Opioid Response Program (RCORP) initiatives. The measures address SUD treatment provision and referral, SUD prevention and recovery outcomes, healthcare provider and community member education, opioid-related overdoses and mortality, and consortium sustainability and strength. Comments are due August 26, 2019.

3 On Your Side Investigates: Emergency Delayed
Jul 24, 2019 -- In Mississippi, hospital closures leave rural residents relying on emergency medical services that may have to travel long distances due to county contracts that opt for less expensive services to balance fewer taxpayers. An analysis found that 1/5 of the state's counties do not have an emergency room and of those, EMS response times ranged from an average of 14 minutes to a high of 78 minutes; more than half of the counties that responded did not have EMS response time requirements.

After Nursing Home Closes, Rural New Mexico City Searches for Solutions
Jul 24, 2019 -- In rural Lordsburg, New Mexico, the community's only nursing home closed, forcing its 37 residents to relocate and costing the community about 80 jobs. The community was surprised by the closure and is looking for answers in the midst of the emotional and economic impact of the nursing home's closure. (May require subscription to view).

New Resources

2020 Census Predictive Models and Audience Segmentation Report: A New Design for the 21st Century
Reports on how different segments of the population are likely to self-respond to the 2020 Census. Identifies eight audience segments, groupings of census tracts with similar predicted self-response behavior and similar demographics related to self-response. Includes a map showing how they are geographically distributed. Two of the segments, Country Roads and Sparse Spaces, are rural. Others include rural areas. For each audience segment, information is provided on demographics, media consumption, and mindset related to the census.

Immigrant Doctors Can Help Lower Physician Shortages in Rural America
Report highlighting physician shortages in rural areas and exploring pathways by which immigrants come to practice in those areas. Provides recommendations for state and federal policies that would better incorporate immigrant doctors into the effort to ease healthcare shortages in rural areas. Medicaid: States' Use and Distribution of Supplemental Payments to Hospitals
Examines the use of disproportionate share hospital (DSH) payments, which are given to hospitals with more Medicaid and uninsured patients. Addresses differences in state Medicaid expansion status, uninsured percentage, and uncompensated care in relation to DSH payments. Includes state data on DSH payments and uncompensated care costs by rural/urban hospital location, as well as data on Critical Access Hospital (CAH) and Sole Community Hospital DSH payments by state.
