  • Need: Before 2000, Butte and southwest Montana had around 1,300 cases of child abuse a year, with only a 20% conviction rate for perpetrators of sexual abuse.
  • Intervention: Multiple agencies in the community came together to address the issue of child abuse by forming the Butte Child Evaluation Center (CEC), a Children's Advocacy Center.
  • Results: During a 3-year grant cycle, over 200 interviews and exams were performed on victims of sexual abuse and the Butte CEC became the first program in Montana to be accredited by the National Children's Alliance.

  • Need: To reduce 911 use and improve older adults' health in rural Santa Cruz County, Arizona.
  • Intervention: Community paramedics made scheduled visits to patients and connected them to other community resources.
  • Results: CHIPP assisted over 150 people, and 911 calls decreased.

  • Need: In rural Garrett County, Maryland, a 1998 community survey found that 41% of kindergarten students had untreated dental decay, and many dentists in the area were not willing to see patients with medical assistance or state health insurance plans.
  • Intervention: In 1999, the Garrett County Health Department started the Something to Smile About program to improve access to dental care and help dentists negotiate higher reimbursement rates from managed care organizations.
  • Results: The program established a community dental clinic, provided care to thousands of individuals, and negotiated 30% higher rates for dental service reimbursement.

  • Need: Dental care access for children in low-income families living in a 7-county region of southwest Missouri.
  • Intervention: A mobile dental unit was created to expand dental care access.
  • Results: The Miles for Smiles mobile dental clinic provides comprehensive dental care to children throughout the 7-county region.

  • Need: To access specialty medical services, residents of Santa Catalina Island have to travel 20 miles by boat to southern California. A telemedicine service was needed to connect island residents to the mainland's specialty services.
  • Intervention: The Catalina Island Medical Center created a telemedicine center to give residents quick access to specialty medical care, including diabetic consultations and pain management.
  • Results: An analysis of the telemedicine program found that patients had measurable improvements, and that 90% of patients were pleased with the program and will continue to use it.

  • Need: Connect individuals to services that address health barriers.
  • Intervention: A pay-for-outcomes model utilizing Community Health Workers who help provide community members with tools to address needs associated with improving health.
  • Results: Trained Community Health Workers help patients navigate the healthcare and social service systems and provide education about community healthcare resources.

  • Need: Low rates of immunization and a lack of knowledge about physical health among school age children in the rural areas of Nelson County, Virginia.
  • Intervention: A School Nurse Program placed a registered nurse in each of the four county public schools to track and encourage immunization compliance, provide health education, and handle students' daily health issues.
  • Results: School-age children are having many of their minor health concerns addressed throughout the day by registered nurses at school. Compliance for childhood immunizations is now extremely high.

  • Need: Comprehensive cancer services for residents of an 8-county, 3-state area in Appalachia.
  • Intervention: Using a Cancer Patient Navigation Tool Kit, a Maryland acute care facility led a multidisciplinary collaboration that provided the area's patients with expanded cancer treatment services.
  • Results: In addition to several new cancer-related programs, expanded services are now available for cancer patients, families, and cancer survivors.

  • Need: To help reduce diabetes, depression, and stroke risk in rural residents.
  • Intervention: A collaborative care model was implemented in the Idaho counties of Clearwater, Idaho, and Lewis.
  • Results: Increased number of patients with controlled blood sugar, controlled blood pressure, and higher depression screening rates.

  • Need: Decrease hospital readmissions and emergency room visits for patients in rural Tidewater, Virginia.
  • Intervention: After inpatient admission or ER visit, identify at-risk patients to offer enrollment in remote monitoring and disease self-management education and coaching, with a special focus on behavioral health wellness.
  • Results: Decreased readmissions and ER visits paired with high patient satisfaction scores.
