Module 2: Program Models

There are several types of community health worker (CHW) program models. These models vary based on the services provided, the patients and community members they serve, and the goals of the health program, among other factors. This module provides information describing the different types of CHW program models, how the models serve rural communities, and how the models can be adapted to meet community needs. There are also resources related to the differing CHW models.
