Healthy Heart Toolkit

Shares a variety of resources designed to help Native communities implement a cardiovascular disease risk reduction program. Based on the strategies used by the Special Diabetes Program for Indians (SDPI) Healthy Heart (HH) grantees, the toolkit can be adapted to best suit community needs and provides guidance in the areas of partnership development, participant recruitment, staffing, messaging, and more.
