Patient-Centered Medical Home Services in 29 Rural Primary Care Practices: A Work in Progress
Examines the progress of rural primary care practices as they implement the processes and services of Patient Centered Medical Homes (PCMHs) across five domains: access to care, population-based care, quality, care management, and clinical information management. Findings are based on responses from a national survey of 29 rural physician practices.
