The Community Health Matching program offers grants to nonprofit community organizations that develop, implement and evaluate new approaches to healthcare issues.

Proposals are reviewed on the criteria:

  • Project sustainability
  • Potential for replication
  • Significance and risk-level of the target population
  • Level of impact on Michigan health and healthcare

Nonprofit 501(c)(3) organizations based in Michigan are eligible to apply.

Amount of funding

Up to $25,000 per year for two years.

Matching funds are required based on the size of the applicant's budget and the percentage of Medicaid and uninsured patients it serves. For complete details, see the program website.

Potential applicants are encouraged to submit a concept paper to for feedback before submitting a full proposal.

Applications are accepted at any time and reviewed at Board of Directors meetings 3 times per year. Review dates and deadlines are listed on the program website.
