Minnesota Department of Health - Office of Rural Health and Primary Care
Minnesota Rural Health Awards honor individuals and teams
for outstanding contributions to rural health in
Minnesota. Awards fall under the categories:
- Hero Award - an individual
- Team Award - a team or group of people
- Lifetime Achievement Award – an individual at the
“end” of their career
Nominees may have contributed to rural health in any
capacity: through a volunteer or paid position, as a
healthcare provider or non-provider, in policy or through
practice. The Lifetime Achievement Award is intended to
recognize individuals who, over the span of their career,
have made significant, lasting contributions to improving
rural health in Minnesota.
Areas of impact could include but are not limited to:
- Rural health policy
- Patient care
- Healthcare systems
Awards will be announced on National Rural Health Day,
which is November 16, 2023.
Any individual or team is eligible if they have
contributed to rural health in the state.
Amount of funding
These are honorary awards.
