Minnesota Department of Health - Office of Rural Health and Primary Care
The Minnesota Office of Rural Health and Primary Care
coordinates a state application to HRSA Federal Office of
Rural Health Policy on behalf of the needs of Minnesota's
small rural hospitals. Funds may be used to purchase
items from the SHIP Purchasing Menu in the categories of
value based purchasing, accountable care organization,
and payment bundling.

The estimated maximum award for each hospital is $13,312.
The estimated grant period is July, 2024 to May, 2025.
