The Minnesota Office of Rural Health and Primary Care coordinates a state application to HRSA Federal Office of Rural Health Policy on behalf of the needs of Minnesota’s small rural hospitals. Funds may be used to purchase items from the SHIP Purchasing Menu in the categories of value based purchasing, accountable care organization, payment bundling, or care transitions.

Specific funding priorities and ineligible expenses are listed in the application instructions.


Small rural hospitals in Minnesota with 49 or fewer beds are eligible to apply.

Grants are expected to be between $8,000-$9,000 depending upon the number of hospitals that apply.

Links to the application instructions and the application form are available on the program website. Allowable items from the Purchasing Menu are listed on the application form.

To be included in the state consolidated application, hospitals must email the completed form to Judith Bergh by January 8, 2018.
