Minnesota Small Hospital Improvement Program (SHIP)
The Minnesota Office of Rural Health and Primary Care coordinates a state application to HRSA Federal Office of Rural Health Policy on behalf of the needs of Minnesota’s small rural hospitals. Funds may be used to purchase items from the SHIP Purchasing Menu in the categories of value based purchasing, accountable care organization, payment bundling, or care transitions.
Specific funding priorities and ineligible expenses are listed in the application instructions.
Small rural hospitals in Minnesota with 49 or fewer beds are eligible to apply.
Applicants can request $12,000. The actual award is estimated to be between $10,000 - $12,000, depending on the number of hospital applications nationwide.
The estimated grant period is June 1, 2019 – May 31, 2020.
To be included in the state consolidated application, hospitals must email the completed form to email@example.com by November 30, 2018.
