Purpose

The Minnesota Office of Rural Health and Primary Care coordinates a state application to HRSA Federal Office of Rural Health Policy on behalf of the needs of Minnesota's small rural hospitals. Funds may be used to purchase items from the SHIP Purchasing Menu in the categories of value based purchasing, accountable care organization, and payment bundling.

Eligibility

Small rural hospitals in Minnesota with 49 or fewer beds are eligible to apply, including Critical Access Hospitals.
