Purpose

Healthcare Workforce Collective Impact Grants provide funding for community coalitions to identify and address a pressing community health issue through changes in policies, norms, practices, social supports, and the physical environment.

Focus areas may include but are not limited to:

  • Heart disease
  • Diabetes
  • Mental health
  • Obesity
  • Disparities in access, quality or cost
  • Healthcare workforce development
Eligibility

Louisiana-based 501(c)(3) nonprofit organizations in good standing with the Louisiana Secretary of State are eligible to apply.

Amount of funding

Award ceiling: $1,000,000
Award floor: $250,000
Project period: 3 years

A one-to-one matching contribution is required.

Submit an initial letter of intent through the online grant portal. Selected applicants will be invited to submit a formal application.
