Oregon Rural Population Health Incubator Program

The Population Health Incubator Program provides funding and technical assistance to support rural population health programs initiated by rural hospitals, clinics, public health departments, or community-based organizations in Oregon.
Program goals include:
- Provide funding to create or maintain an innovative programs that support needs around population health and address key social factors that impact well-being
- Prioritize funding to serve high-need areas and populations; identify pressing community health needs and work with multiple community partners to address the identified need
- Provide coaching, mentorship, referrals to experts and other resources
- Provide a supportive and collaborative environment for grantees to share knowledge and learn from one another
Eligible organizations are hospitals, clinics, public health departments, and community based organizations located in a rural area in Oregon that are implementing a program aimed at increasing community health, whether directly or indirectly.
Awardees will receive a grant of up to $10,000 and specialized support, including resources, information and referrals to experts to assist their program.
