Purpose

This Health Center Program School-Based Service Expansion (SBSE) provides grants to add new Health Center Program service delivery sites at schools or expand comprehensive primary healthcare services at existing Health Center Program service delivery sites located at schools.

Applicants must increase the number of patients served at school-based service sites by adding or expanding one or more of the following services at a new or existing school-based service site:

  • General primary medical care
  • Mental health
  • Substance use disorder
  • Oral health
  • Vision
  • Enabling services

Applicants must either currently provide mental health services or propose to expand mental health services at each school-based service site included in the application.

Funds may also be used to support one-time costs related to equipment purchases and minor alterations and renovations necessary for increasing the number of school-based service site patients

Eligible applicants are existing Health Center Program operational (H80) grant award recipients.
