This program provides grants to add new Health Center Program service delivery sites at schools or expand comprehensive primary healthcare services at existing Health Center Program service delivery sites located at schools.
Applicants must demonstrate how projects will address unmet needs by adding or expanding one or more of the following services at a new or existing school-based service site:
- General primary medical care
- Mental health
- Substance use disorder
- Oral health
- Vision
- Enabling services
Funds may be used to support varied operational costs, including personnel and supplies, as well as one-time costs related to:
- Equipment purchases, including mobile units and telehealth infrastructure
- Minor alteration and renovation necessary for increasing the number of school-based service site patients
Eligible applicants are existing Health Center Program operational (H80) grant award recipients.
Award ceiling: $200,000 per year
Project period: 2 years
