The Produce Prescription Pilot Program (P4) will provide funding to establish or expand programs that connect healthcare providers with food systems to provide fruits, vegetables, and traditional foods to eligible participants. Programs are expected to integrate clinical services with community-based food access strategies and community appropriate nutrition education that reflect community values and practices.
Recipients will develop partnerships with healthcare providers, community organizations, and food suppliers to implement sustainable produce prescription models. Programs should prioritize populations experiencing food insecurity and chronic conditions influenced by food access and nutrition such as diabetes, while also supporting broader physical, social, and whole person well-being.
The goal of the pilot is to demonstrate and evaluate the impact of Produce Prescription Programs on AI/AN people and their families, specifically by:
- Reducing food insecurity
- Improving nutrition and dietary patterns by increasing access to and consumption of fruits, vegetables, and traditional foods, while building nutrition knowledge through education that supports healthier food choices and long-term behavior change
- Promoting whole-person health and well-being

Eligible applicants include:
- Federally recognized tribes
- Alaska Native villages, groups, and regional/village corporations
- Tribal organizations
- Urban Indian organizations
Award ceiling: $250,000 per year
Award floor: $200,000 per year
Project period: 5 years
Estimated number of awards: 14-18
Estimated total program funding:
$3,500,000
