Purpose

This opportunity (Short Title: Prevention Navigator) will provide funding to provide substance use and HIV prevention services for racial and ethnic minority populations at high-risk for substance use disorders (SUDs) and HIV infection.

Required activities:

  • Conduct a needs assessment identifying the population of focus, including racial/ethnic minority populations vulnerable to SUD, HIV, and Viral Hepatitis
  • Develop and enhance local capacity and mobilization of community resources to implement culturally and linguistically effective programs, practices, and policies to prevent the onset of SUD, reduce sexual risk factors and the spread of HIV/AIDS among high-risk racial/ethnic minority populations
  • Develop a data-driven, comprehensive strategic plan based on the results from the needs assessment that is culturally and linguistically age-appropriate to include:
    • Documented population needs, risk and protective factors, including appropriate evidence-based/informed programs, practices, and strategies
    • Proposed direct and indirect prevention interventions, practices, and strategies each year and over the 5 year project period
    • Proposed evidence-based programs that will be used, how outreach will be conducted, and whether HIV, Viral Hepatitis, and STI testing strategies will be conducted on-site or through referrals; and identify appropriate social media and social marketing campaigns, including educating providers and others on substance use and HIV, Viral Hepatitis, and STI infections
    • Proposed type and frequency of all activities that will be conducted each year and over the life of the project, including a list of organizations with an established partnership for referral services
  • Implement evidence-based programs, practices, and strategies for the population of focus, including:
    • Implement substance use, HIV, and Viral Hepatitis evidence-based prevention services, including environmental strategies to change community norms
    • Provide extensive outreach that includes recruitment and retention of participants for the prevention program
    • Provide screening and risk assessments for substance use, HIV, and Viral Hepatitis, including purchase of HIV and Viral Hepatitis test kits and pre- and post-test counseling
    • Provide navigation services to link individuals to support services, including housing, mental health, substance use treatment, and follow-up
Eligibility

Eligible applicants are domestic public and private nonprofit entities located in states and communities hardest hit by the HIV Epidemic (the 48 counties; San Juan, Puerto Rico; and Washington, D.C.) and seven states with a substantial rural burden as outlined in the National HIV/AIDS Strategy for Ending the HIV Epidemic: A Plan for America (EHE).
