Purpose

The Minnesota Department of Human Services is offering grants to increase the number and capacity of home and community-based services (HCBS) providers in rural and/or serving rural or underserved communities in Minnesota.

Grantees must use one of the waivers or programs listed in Section 2 (General information) of the application instructions.

Funding may be used to build organizational capacity in one or more of the focus areas:

  • Program compliance and quality improvement - Developing internal tools, processes, policies, and procedures to ensure service delivery meets regulatory standards and supports continuous quality improvement
  • Workforce development - Addressing workforce shortages through professional development and training that enhances staff expertise, improves program implementation, and strengthens organizational management capacity
  • Person-centered practices - Building capacity to integrate person-centered philosophies, ensuring individuals have meaningful choice and control in their lives
  • Organizational infrastructure - Improve or upgrade internal systems such as accounting, human resources, billing, and administrative management tools to deliver effective services
  • Technology and equipment - Purchasing technology, software, or other equipment to support community-based services, enhance efficiency, and ensure compliance with Electronic Visit Verification (EVV) requirements
  • Accessible transportation - Purchasing or modifying vehicles to provide safe, individualized transportation for HCBS participants
  • Facility and home modifications - Making modifications to buildings or homes to meet participant needs and improve accessibility
  • Assistive technology - Acquiring equipment, products, or systems such as wheelchair ramps, vehicle platform lifts (VPL), ceiling lifts, or other assistive technologies to increase, maintain, or improve functional capabilities for HCBS participants
Eligibility

Applicants must be currently licensed and enrolled in Minnesota Health Care Programs (MHCP), actively providing services, and committed to improving, enhancing, or strengthening the infrastructure used to deliver HCBS.

Applicants must be located and serving clients in Minnesota.
