Purpose

The Tennessee Department of Health is offering grants to develop or expand innovative perinatal telehealth models that strengthen Tennessee's maternal health system, reduce gaps in access to specialty obstetrical care, and support a more coordinated statewide approach to high-risk perinatal services. Projects should serve as building blocks toward a more unified perinatal telehealth system capable of improving patient outcomes, decreasing travel burdens, strengthening referral pathways, and supporting providers in rural and healthcare shortage communities.

Required activities:

  • Real-time audio/visual telehealth consultations for high-risk obstetrical care
  • Provider-to-provider consultation or case review
  • Care coordination for high-risk prenatal and postpartum patients
  • Patient and provider education on accessing and using telehealth
  • Demonstrated alignment with Regional Perinatal Centers (RPC), hospitals, clinics, local health agencies, and other relevant partners
  • Establish a formal partnership with Maternal–Fetal Medicine (MFM) specialists or an MFM practice serving the proposed geographic area
  • A detailed engagement plan for each key partner
  • A comprehensive evaluation plan to collect visit-level and outcome data
  • Participation in state-required evaluation processes
Eligibility

Eligible applicants are agencies and organizations with the experience and ability to establish/expand perinatal telehealth services that target underserved populations.
