Outpace CVD Rural Midwest Healthcare Grant

Purpose

The American Heart Association (AHA) is offering grants for outpatient care sites in the rural Midwest that are working to manage chronic disease and prevent cardiovascular disease.

Funding may be used to:

  • Accelerate work in a comprehensive approach to managing hypertension and/or Type 2 diabetes through a focus on one or more of the strategy pillars identified by the American Heart Association
  • Develop or enhance collaboration with external partners to systematically and sustainably address chronic disease directly and/or move upstream to address drivers of health such as tobacco cessation and nutrition security

Selection criteria:

  • Commitment to register for at least one new component of Outpace CVD (or continue participation in all 3 programs as in the past) and submit all required summary level data by May 1, 2026
  • Interest and commitment to implement one or more new evidence-based clinical policy or practice change in consultation with AHA staff
  • Readiness and capacity to submit data
Eligibility

Eligible applicants are outpatient care sites in the rural Midwest including, but not limited to, health systems, Rural Health Clinics, Federally Qualified Health Centers, and safety-net free clinics.
