Purpose

The North Carolina Rural Organizations Orchestrating Transformation for Sustainability (NC ROOTS) Hubs grant provides funding for regional, locally governed networks that coordinate services and lead the implementation of North Carolina's Rural Health Transformation Program (NCRHTP) initiatives. Working with community partners, each Hub will help advance efforts across primary care, behavioral health, workforce development, value-based payment, and digital health while tailoring strategies to the unique needs of their region.

Each NC ROOTS Hub will establish and coordinate a regional network of experts and partners to advance program initiatives. This network may include hospitals, local health departments, community-based organizations, providers, and other stakeholders working collaboratively to design and implement rural health improvement projects that respond to the needs of rural residents.

Up to six organizations will be selected, one for each North Carolina Medicaid region. Applicants must demonstrate the infrastructure, knowledge, experience, and capacity to serve as both the programmatic and fiduciary lead for their region, guiding a partner network to successfully implement NCRHTP initiatives.

Eligibility

Public and private entities with at least 3 years of office presence located in and experience delivering programs in North Carolina, including tribal organizations, are eligible to apply. Applicants must have sufficient capital and fiduciary capacity to cover program costs before receiving grant funds.

Amount of funding

Funding for the initial grantee award period is $39,252,177 for each of the 6 regions. The initial contract will run between June 1, 2026 through October 30, 2027. Projects will renew on an annual basis through the total project period as funding allows.
