Environmental Justice Small Grants Program - Tribal Governments Inactive
Grants to support community-driven projects designed to engage, educate, and empower communities to better understand local environmental and public health issues and develop strategies for addressing those issues, building consensus in the community, and setting community priorities. Projects should help residents of underserved communities understand and address local environmental and public health issues. The 2022 opportunity is limited to federally recognized tribal governments to establish or modify public participation programs where fair treatment and meaningful participation priorities have been impacted by the COVID-19 pandemic.

Utah Multicultural Rural Mental Health Grant Inactive

The population of focus is individuals of racial and ethnic minorities with a serious mental illness or co-occurring disorder living with or at risk for HIV and/or hepatitis.

Accountable Care Organization Realizing Equity, Access, and Community Health (ACO REACH) Model Inactive
Payment model to test financial risk sharing options, as well as innovative approaches from Medicare Advantage (MA) and private sector risk sharing arrangements. ACO REACH seeks to improve quality of care and health outcomes for Medicare beneficiaries through alignment of financial incentives to promote effective and appropriate care; the promotion of health equity among all model participants; emphasis on patient choice; strong monitoring to ensure that beneficiaries maintain access to care; and emphasis on care delivery for complex, chronically, and seriously ill populations.

Funding to improve access to and quality of substance use disorder and other behavioral healthcare services in rural communities including utilization of prevention, treatment, and recovery services to improve the care for those affected by behavioral health conditions.

Funding for small, rural Local Education Agencies for initiatives aimed at improving student academic achievement. Activities may include drug prevention, violence prevention, school-based health services, and other health-related programs.
