Columbia Gorge Collective Impact Health Specialist

  • Need: To address the specific health needs of north central Oregon and south central Washington.
  • Intervention: The Collective Impact Health Specialist identifies community needs, convenes community partners to design initiatives that address those needs, and secures funding for health-related initiatives.
  • Results: Thanks to the CIHS, the Columbia Gorge region has received $20.5 million since 2014.
Rural Libraries and Health Cooperative Agreement

  • Need: To increase access to healthcare and social services in rural South Carolina via library systems.
  • Intervention: Pilot sites are hiring social workers and community health workers to provide consultations, case management, and referrals.
  • Results: One library's social worker program currently has 62 active clients, and another library screens an average of 215 residents a month.

  • Need: To reduce deaths due to opioid overdose in rural southeast West Virginia.
  • Intervention: The Community Connections, Inc. Rural Health Opioid Program trained and certified first responders, healthcare staff, and laypeople in naloxone administration.
  • Results: From October 2018 to September 2021, the program provided direct education to 638 individuals, trained 821 providers, and distributed 4,023 Narcan kits.
Tea Time with Teens

  • Need: To lower teen pregnancy rates in Marlboro County, South Carolina, and develop teen leaders.
  • Intervention: Tea Time with Teens brings together community leaders, mothers, and daughters to build life skills and make healthy decisions.
  • Results: Since 2009, the program has been educating teens on making healthy choices and adults on having meaningful conversations with teens.
