Purpose

The Million Hearts Hypertension Control Challenge recognizes clinicians, practices, and health systems that have made a significant impact on hypertension rates within their communities, with a goal of preventing one million heart attacks and strokes by December 2026. Priority is given to communities that experience inequities.

Eligibility

The Challenge is open to public and private clinicians, medical practices, and health systems in the United States. See the program website for complete eligibility guidelines.

Amount of funding

Up to 35 of the highest scoring applicants will be recognized nationally as Million Hearts Hypertension Control Champions. No cash prize will be awarded.
