Purpose
The Minnesota Office of Rural Health and Primary Care provides grants to Federally Qualified Health Centers (FQHCs) and FQHC Look-Alikes operating in Minnesota to continue, expand, and improve services to low-income populations.

Eligible participants include FQHCs and FQHC Look-Alikes operating in Minnesota.

Applications for step 1 are due September 26, 2022. Step 1 will be used to determine eligibility and award distribution amounts. Narrative and budget applications must be submitted online by November 14, 2022.
